Your First Visit

Dr. Burkhart treats many chronic digestive conditions. She has particular expertise in SIBO, Celiac disease, Gluten/wheat related disorders, IBS, food allergies and sensitivities, the Low FODMAP diet and Dysautonomia/POTS. The initial appointment is 2 hours. During this time you will discuss your medical history/health concerns, diet, lifestyle, as well as sleep, community, social support system, habits etc. The goal of your first visit is for Dr Burkhart to get a comprehensive overview of you and your health concerns. Based on the visit she will make recommendations based on your needs and beliefs. For some patients this may include supplements, and complementary therapies. For others it may be more conventional approaches such as dietary and lifestyle guidelines and medications. Dr Burkhart is happy to provide options to fit your individual needs.

Forms

All patients must complete a New Patient Questionnaire prior to your initial visit. This provides Dr. Burkhart with the information necessary to understand your condition and how she can best help you.The new patient questionnaire will be sent electronically at the time of scheduling.   • *Please submit your health questionnaire electronically at  least one week ahead of you appointment date. If you choose to complete a paper copy, use the link here or below. For a paper copy, you may mail it or drop it by the office at any time. There is a locked drop box at the door.
  • *If we have not received your questionnaire prior to your appointment we will ask that you reschedule your appointment.
  • *If you are attempting to get an appointment sooner than is currently available, please complete the questionnaire as soon as possible. Priority for cancellation spots is given to patients with a completed questionnaire.

Due to health privacy rules Dr. Burkhart is unable to receive questionnaires via email.

Fees/Insurance Coverage

The initial consultation fee includes the 2 hour appt. and the time Dr. Burkhart spends on pre-appointment research and preparation. Since many of her patients have complicated medical histories, Dr. Burkhart likes to review all cases ahead of time. If questionnaires are not received prior to the initial appointment we will ask that you reschedule the appointment. The initial health questionnaire is very comprehensive. Submitting it ahead of your appointment date allows more time to discuss your unique situation with Dr. Burkhart during the actual visit.The questionnaire is located here.or at the link below..

She does not participate with any insurance plans and does not bill insurance directly but will gladly provide you with a completed form to send to your insurance company for possible reimbursement. Many insurance companies will provide at least partial reimbursement for the consultation and full coverage for any laboratory testing. Please check with your insurance provider to verify details.

Please remember this is a consult service, and that it is imperative for you to continue your treatment with a primary care physician for routine care and emergencies.

Appointments

All appointments are currently being done virtually due to the COVID-19 pandemic. New patients must at this time, reside in either California or Hawaii, due to federal practice guidelines. Once she begins to see patients in the office again , or if regulations change prior, she will be able to see any new patient, regardless of state of residence.
