Fees/Insurance Coverage

Dr. Burkhart is aware of the costs of treatment and focuses on keeping costs as low as possible for her patients. Any expense for testing and therapies will be discussed together with her patients prior to administration. She does not participate with any insurance plans and does not bill insurance directly but will gladly provide you with a completed form to send to your insurance company for possible reimbursement. Many insurance companies will provide at least partial reimbursement for the consultation and full coverage for any laboratory testing.  Please check with your insurance provider to verify details.

The initial consultation fee includes the 2 hour appt. and the time Dr. Burkhart spends on pre-appointment research and preparation. Since many of her patients have complicated medical histories, Dr. Burkhart likes to review all cases ahead of time. Please submit your health questionnaire via fax: (707) 927-5747 or mail: 1100 Lincoln Ave. Suite 200, Napa, CA 94558 at least one week ahead of your appointment date. You may also drop it by the office at any time. There is a locked drop box outside our door. Due to health privacy rules she is unable to receive questionnaires via email. If questionnaires are not received prior to the initial appointment we will ask that you reschedule the appointment. The initial health questionnaire is very comprehensive. Submitting it ahead of your appointment date allows more time to discuss your unique situation with Dr. Burkhart during the actual visit.   The questionnaire is located under “New Patient Information”.

Please remember this is a consult service, and that it is imperative for you to continue your treatment with a primary care physician for routine care and emergencies.

Telephone Appointments

Dr. Burkhart is happy to conduct follow-up telephone appointments if you have seen her in person for an initial visit. The telephone appointments are charged at the same rate as an office visit.

Cancellations or Missed Appointments: If you are unable to make your appointment, we ask that you cancel your appointment at least 24 hours in advance.
